ANDALUSIA HEALTH AND FITNESS LLC — WAIVER, RELEASE OF LIABILITY AND COVENANT-NOT-TO-SUE
I UNDERSTAND AND AGREE that Andalusia Health and Fitness LLC is not responsible for injury or loss of property while any person participates in activities at the facility, including ordinary negligence by Andalusia Health and Fitness LLC or its agents.
I understand the facility may be unsupervised during non-staff hours; I use it at my own risk and will not allow non-members to enter. Enforcement may include electronic surveillance and fees or termination for violations.
I agree to consult a physician and understand the Club bears no responsibility for my physical health. In consideration of access, I agree not to sue and release the Club from any and all claims arising from participation, including ordinary negligence.
I acknowledge risks associated with weight training, equipment use, and actions of other participants, and agree to use safety equipment properly. This waiver is intended to be as broad and inclusive as permitted by Alabama law. Full terms of the Membership Agreement apply.
